As a community health center, we provide personalized care to help manage our patients’ individual and family wellness needs through all stages of life. Our goal is to provide a medical home that focuses on high quality primary medical care, walk-in services, dental, pharmacy, and outreach services for the whole family.
We have three school-based health centers at Evergreen Elementary, Lorna Byrne Middle School and Illinois Valley High School. We offer primary care services located within the school. Our school-based health centers can examine, diagnose and prescribe treatment for children while they are at school.
Siskiyou Community Health Center is also proud to be an Authorized Urgent Care Provider for Veterans. This program provides veterans with non-emergency care in our Walk-In Clinic quickly and conveniently, without pre-authorization.
